The Profile Information field should be used to collect information on the primary submitter and contact on a form submission or event/group registration.
If you would like to capture multiple individuals' contact info on a form, use the People field.
This button represents a collection of personal information fields that can be included in the form. It is recommended that this be added to every form you design, as it contains fields that help identify who is submitting the form. Click this button to see the profile information pop-up window.
1. Include Profile Fields?
-This field enables profile fields for the form. Select “Yes” to view available fields. Select "No" to disable these fields. This will allow the form to be submitted anonymously.
2. Enable Login?
- This field determines whether the login button is displayed on a form. The benefit of allowing members to log in directly on the form page means the submission will automatically be assigned to thier profile and they can use saved payment methods if applicable.
3. Create new profile if submission cannot be matched to an existing person profile?
- Check this box to allow the system to create a new profile if no match is found. Note the system will match submissions to profiles based on Name + Email or Name + Phone number.
4. Add person to guest list if form is linked to an event?
- If the form is linked to an event, enabling this will automatically add the individual to the event’s guest list. You may not want the submitter to be added to the guest list if the event is only for minors, but the parents are submitting the form.
5. Person Profile Fields
- Check the "Show" column to include the field.
- The "Required" column if the person must provide a value when submitting the form.
- The "Update" column will allow the information to be added to a profile if the matching field is empty.
- The "Overwrite" column will allow the information to be updated in a profile, removing the existing data.
Name - adds first and last name fields (must be included and required)
Email - adds a field to collect email. The format is enforced.
Phone - adds a field for the phone number.
Note: when a mobile number is selected in the Phone field, a question will populate asking if the member wants to opt-in to text messages.
Campus - adds a dropdown for campus
Address - adds fields for each part of the person's address
Birthday - adds a dropdown for the date of birth
Gender - adds a dropdown for the gender
Marital Status - adds a dropdown for the person to specify their marital status
Contact Preference - adds a dropdown for contact preference
