How to create a VBS registration form?

Describes how to design a Vacation Bible School form.

Forms are a powerful tool to streamline various aspects of church administration, including planning and managing Vacation Bible School (VBS). Such a VBS form can efficiently collect participant information.

Create a form

  1. On the left-hand menu, click on Form > Add New or on the top page

    OR go to "+ New" on the top of the page > Form.

  2. Name your form a clear and descriptive title, such as "Vacation Bible School Registration Form."

  3. Select the "Create" button.

  4. Click on "(add description)" and then add a brief description to provide context for the form's purpose.

  5. Choose the "Done" button.

Add parent/guardian information fields

  1. Click on the Person Profile Information field.

  2. Checkmark the following:

    1. "Create new profile if submission cannot be matched to an existing person profile?"

    2. If you would like the parent added to the guest list, check

      "Add person to guest list if form is linked to an event?"

  3. Ensure the field for "Email" and mark it as "Required" and "Update".
    i. This will help to ensure you have info for communication and confirmation

  4. Checkmark the field for "Phone" and mark it as "Required" and "Update".

  5. Check additional desired fields.

  6. Click on "Done."

Optional: Add item/ticket field

Including an item/ticket will allow you to charge a fee per child if necessary for meals or shirts.

OR

If you need to set a limit on the number of attendees, you can limit the ticket quantity and set it to free.

  1. Add an item/ticket field and, as an example, put "Children Tickets" as the Label.

  2. Select Add item, enter the name, and then the total quantity if there is a limit

  3. If there is a fee, add the price. If the event is free, set it to $0.00

  4. Click "Done" to save the ticket.

  5. Click "Done" to save the field

Add People field for children information

  1. Choose the People field

  2. Label as "Children" and mark it as "Required."

  3. Set the "Person Label" to Child.

  4. Set the "Position" to "Child".

  5. Check the box to link to the created ticket

    Note: This will automatically add a ticket per child added to the form

  6. Click on the Name field in the Fields section.

  7. Checkmark the following:
    "Create new profile if submission cannot be matched to an existing person profile?" and "Add person to guest list if form is linked to an event?"

  8. Choose the "Birthday" field as required and "Update".

  9. Choose the "Gender" field as required and "Update".

  10. Click "Done" to save the child information field.

Add other fields for each child:

Allergies

  1. At the bottom of the people field, click on the "Add new field" button

  2. Select the appropriate Type; in this case, paragraph.

  3. Type in "Allergies" as the Label.

  4. Set the field as "Required".

  5. Then click on "Done."

T-shirts

  1. Click on the "Add new field" button and select the appropriate Type; in this case, dropdown

  2. Type in "T-Shirt Size" as the Label.

  3. Then scroll down and click on "Bulk add" to input choices for which people can choose, such as Small, Medium and Large

  4. Click on "Done."

  5. Set the field as "Required".

  6. If everything looks good, select "Done."

Save the form

Click the "Save" button to finalize the form.

Review the form

Examine the form to ensure all fields are correctly displayed. After you have created your form, you can share it with your congregation in a variety of ways, such as by embedding it on your website or sending a link to it in an email.

Keep in mind

  • Keep the form concise and user-friendly: Avoid overwhelming users with excessive questions.

  • Use clear and concise language: Avoid jargon or technical terms.
