How to use the Date field

Describes how to use the Date field on forms.

This will add a field that only accepts dates to the form. This is to allow someone to type in a date or select it from a calendar widget. Date format is limited to MM/DD/YYYY.


If you are collecting date of birth, please use the Profile Information field or the People field.


The following fields are available when configuring this field:

Label - The field name

Description - Text entered here will appear between the field name and the text box.

Restrict Dates - Limits which dates a person can enter or select in this field. Choose from:

  • No restriction - Any date can be entered. This is the default.

  • Specific dates - Set an exact Earliest and/or Latest date the field will accept.

  • Relative to today - Set an Earliest and/or Latest allowable date as a number of days, weeks, months, or years before or after the date the form is submitted.

Tip: Use Relative to today when the valid range should always be measured from the current date; as an example, restricting a counseling-request form to dates within the next year. Use Specific dates when the range is tied to a fixed event, like a registration window that opens and closes on set calendar dates.

Automation tab:

You can add an automation by clicking on Add Automation button, and move that person who submitted the form to a group and/or workflow.

Options tab:

Is Required? -Toggle if text must be entered for this field before being allowed to submit.

Admin Only? - If toggled, this field will ONLY appear when editing a previous form submission and is meant to be filled out by an administrator.

Is Conditional? - Makes the visibility of this field dependent on the answer to another field.

Is Hidden? - This will hide the field from the submitter's view. This is needed when a form has submissions with this field already, as those can not be deleted.
